Position Summary:
The Transformative Case Management Program is designed to follow participants on their journey of transformation, using a person-led framework that addresses needs holistically. The program recognizes the influence of the Holy Spirit in providing strength and a desire for change. The Case Worker will work primarily with shelter residents, developing holistic case plans, completing assessments, and implementing, monitoring, and evaluating actions required to meet plan objectives. This role involves meeting people at their point of need to help rebuild, restore, and renew their lives. The Case Worker will report directly to the Manager of Case Management.
Responsibilities:
The key duties include:
Providing services to residents using evidence-based case management approaches, such as strengths-based, empowerment, trauma-informed, and anti-oppressive practices.
Building rapport with residents through a trauma-informed approach.
Collaborating with residents to provide appropriate referrals and interventions according to their goal plans.
Supporting residents with pastoral care, including spiritual discussions, prayer, and sharing the Gospel.
Conducting initial comprehensive assessments of resident needs and support systems for personalized plans.
Providing trauma-informed crisis management and developing crisis and safety plans.
Coordinating and providing care that is safe, timely, effective, efficient, equitable, and person-centered.
Maintaining an assigned caseload with regular follow-up, tracking case progress, and completing required reporting.
Performing other duties as assigned.
Required Experience and Education:
Undergraduate diploma or degree in social services, social work, or related field, or equivalent combination of education and experience.
Committed follower of Jesus with a vibrant and growing faith.
Excellent holistic assessment, planning, organizational, and administrative skills.
Attention to detail and ability to function well with colleagues and residents.
Knowledge and experience in providing pastoral care, supportive counselling, advocacy, and crisis planning.
Excellent communication skills, both verbal and written.
Familiarity with evidence-based case management practices and approaches, such as trauma-informed care and the Social Determinants of Health and Motivational Interviewing.
Understanding of documentation related to the resident journey, including tracking and reporting.
Understanding of mental health, poverty, trauma, addictions, housing, and homelessness issues in Toronto.
Extensive knowledge of local community resources and the social services landscape in Toronto.
Current Health and Safety training, including Standard CPR C, CPI, WHMIS, ADOA, etc.
Proficiency with Microsoft Office Suite (Dynamics, Excel, Word).
Ability to travel to other TSM facilities to support programs as needed.
Combination of education and experience that demonstrates the ability to perform the responsibilities of this position may be considered in lieu of stated education and experience requirements.
