Reports To: Social Worker
Salary: $20-23/hour
About SEEDS
SEEDS (Supporting Every Eve’s Daughter Safely) is a community-based organization dedicated to supporting women impacted by human trafficking and sexual exploitation. We provide trauma-informed, survivor-centered care with a focus on safety, empowerment, and long-term stability. Our work is rooted in compassion, collaboration, and advocacy.
Position Summary
The Case Coordinator plays a key role in supporting residents by providing individualized case management, advocacy, and coordination of services. This role requires a strong understanding of trauma-informed care, excellent organizational skills, and the ability to work collaboratively with internal teams and community partners.
Key Responsibilities
- Provide individualized, trauma-informed case management to participants
- Develop, implement, and monitor personalized service plans
- Conduct intake assessments and ongoing risk/safety planning
- Coordinate access to housing, healthcare, legal, and social services
- Advocate on behalf of participants with community agencies and systems
- Maintain accurate, timely, and confidential case documentation
- Support participants in goal setting, life skills, and empowerment
- Collaborate with internal staff and external partners to ensure continuity of care
- Participate in case conferences, team meetings, and training opportunities
- Maintain professional boundaries while building trusting relationships
Qualifications
- Diploma or degree in Social Work, Social Services, Psychology, or related field
- Minimum 2 years of experience in case management or frontline support
- Experience working with vulnerable populations, ideally in human trafficking, gender-based violence, or crisis support
- Strong understanding of trauma-informed and survivor-centered approaches
- Knowledge of community resources in Waterloo Region
- Excellent communication, organization, and problem-solving skills
- Ability to work independently and as part of a team
- Valid driver’s license and access to a reliable vehicle (preferred)
- Clear Vulnerable Sector Check
