Position Description
Transitional Youth Support Worker
Location: Oakville, ON | 4 Days In-Person, 1 Day Remote
Schedule: Full-Time (35–40 hours/week), 8-hour shifts scheduled between 9:00 a.m. and 8:00 p.m. This role requires flexibility based on program demands, including one evening shift per week and one Saturday per month
Salary: $48,000–$52,000 annually, commensurate with experience, plus benefits

Oasis Youth Care Programs is a registered Christian charity supporting young adults (ages 18–29) at risk of or experiencing homelessness in the Halton and Peel regions. At The Oasis House, we provide housing, life skills training, and mentorship rooted in compassion, structure, and strength-based care.
About the Role
As a Transitional Youth Support Worker, you are a cornerstone of The Oasis House. You will balance professional case management with relational mentorship, supporting residents as they move from housing instability to self-sufficiency. This role is designed for a proactive navigator who can connect youth to community resources and coordinate mentorship supports.
You will play a central role in maintaining the structure, culture, and emotional safety of The Oasis House while contributing to a supportive, purpose-driven home environment.
Key Responsibilities
Individualized Case Planning & Support
- Manage a caseload and develop Individual Transition Plans (ITPs) focused on housing stability, education, and employment.
- Provide daily structure, encouragement, and youth-centred, trauma-informed support.
- Conduct intakes and orientations, helping new residents integrate into the Oasis community.
Community Resource Navigation & Advocacy
- Connect residents to services across Halton/Peel (healthcare, mental health, legal aid, housing, etc.).
- Build relationships with external community partners to support referrals and continuity of care.
- Accompany youth to key appointments to model self-advocacy and support follow-through.
Mentorship Coordination
- Coordinate mentorship programs and match residents with positive community role models.
- Foster a peer mentorship culture within the house to encourage connection and accountability.
- Help bridge residents into professional networks to support career readiness and opportunity.
Life Skills Coaching & Programming
- Facilitate one-on-one and group life skills workshops (budgeting, meal prep, tenant rights, self-care).
- Plan and lead group activities that support connection, spiritual growth, and reflection.
- Support residents in building routines and skills that strengthen independence and stability.
Housing Stability & Crisis Support
- Maintain a safe living environment by upholding house rules with compassion and consistency.
- Complete safety/hygiene checks and address basic home maintenance needs as required.
- Provide de-escalation and immediate support during mental health crises or conflicts.
Documentation & Professional Practice
- Maintain accurate, timely case notes and incident reporting.
- Participate in staff meetings and contribute to program-wide strategy and improvement.
- Maintain professional boundaries while building trust-based, supportive relationships.
Required Qualifications & Skills
- Education: Diploma or Degree in Child & Youth Work, Social Work, or a related Human Services field
- Experience: 3+ years of experience in residential youth support or housing-based settings
- Crisis Training: Current certification in NVCI/CPI and Mental Health First Aid (or willingness to obtain)
- Knowledge: Strong understanding of trauma-informed care, harm reduction, and the developmental needs of young adults (ages 18–29)
- Values Alignment: Alignment with Oasis’s Christian values and comfort working in a faith-based environment
- Compliance: Ability to obtain a clear Vulnerable Sector Check and hold a valid Driver’s License
