Position Description

 Scott Mission was founded in 1941 by Rev. Morris and Mrs. Annie Zeidman as a non-denominational Christian organization. Each year, it offers practical, emotional, and spiritual support to thousands of people who are experiencing poverty and homelessness in the City of Toronto. Position Summary:

The Transformative Case Management Program will follow the participant's pathway on a journey of transformation. Case management is a person-led framework that looks holistically at one’s need in every area of life. The Scott Mission (TSM) Case Management is transformative because it further recognizes the influence of the Holy Spirit in our God-given purposes, who provides strength and a desire for change. Meeting people at their point of need, TSM seeks to help rebuild what was shattered, restore what was devastated and renew the generations mired in hopelessness (Isa. 61; Luke 4:17,18; Matt. 17). So, through His Gift of comfort, the Holy Spirit, we come alongside people. This Case Worker position, will work primarily with shelter residents, developing holistic case plans. The Case Worker will complete assessments, develop a holistic case plan, implement, monitor, and evaluate actions required to meet the plan objectives. This person will report directly to the Manager of Case Management.

Responsibilities:

  • Provide service to residents from evidence-based case management approaches (e.g., strengths-based, empowerment, trauma-informed approaches, and anti-oppressive practice).
  • Build rapport with residents through a trauma-informed approach.
  • Collaborate with residents to provide appropriate referrals and interventions according to their goal plan.
  • Advocate for residents helping them make informed decisions.
  • Support residents with pastoral care (e.g., spiritual discussions at intake and assessment, prayer, helping in discernment of the resident’s purpose, sharing the Gospel).
  • Conduct initial comprehensive assessments of resident needs and support systems for a personalized plan.
  • Provide trauma-informed crisis management to address chronic crisis situations determining a crisis plan (and safety plan, if need be).
  • Build connections with the resident’s formal and informal supports to optimize opportunities for the resident’s goal plan.
  • Facilitate multiple care aspects (case coordination, information sharing, etc.).
  • Coordinating and providing care that is safe, timely, effective, efficient, equitable, and person centered.
  • Maintain assigned caseload with regular follow-up, tracking case progress and completing required reporting.
  • Respond to emails, phone calls or other correspondences in a timely and responsible manner.
  • Network with external stakeholders and industry partners in the community and utilize TSM partners through referrals.
  • Utilize software applications to record, track and report on the resident’s Journey of Transformation
  • Other duties as assigned.

Required Experience and Education:

  • Education level: completion of an undergraduate diploma or degree in social services, social work, or a related field, or an equivalent combination of education and experience.
  • Committed follower of Jesus with a vibrant and growing faith, and demonstrated agreement and commitment to the Mission, Vision, Core Values, and Statement of Faith of TSM.
  • Excellent holistic assessment, planning, organizational and administrative skills, with attention to detail.
  • Demonstrated ability to function well with colleagues and residents.
  • Knowledge and experience of providing pastoral care and supportive counselling.
  • Knowledge and experience of providing advocacy and crisis planning (safety planning an asset).
  • Excellent communication skills verbally and in writing, active listening skills, ability to speak multiple languages an asset.
  • Familiarity with evidence-based case management practice, approaches (e.g., trauma informed, and the Social Determinants of Health and Motivational Interviewing.
  • Demonstrated understanding of documentation related to the resident journey, including tracking and reporting.
  • Demonstrated ability in decision-making and conflict-resolution skills.
  • Demonstrated ability to work autonomously, to mentor, and to show cultural sensitivity.
  • Demonstrated understanding of mental health, poverty, trauma, addictions, housing, and homelessness issues in Toronto.
  • Extensive knowledge of local community resources and the social services landscape in Toronto
  • Current Health and Safety training including Standard CPR C, CPI, WHMIS, ADOA, etc.
  • Proficient with Microsoft Office Suite (Dynamics, Excel, Word)
  • Ability to travel to other TSM facilities to support programs as needed.
  • Excellent ability to multi-task and be flexible in daily routine of work.
  • Team player with demonstrated problem-solving abilities and willingness for professional development.
  • Knowledge of housing and recovery programs an asset.

A combination of education and experience that demonstrates the ability to perform the responsibilities of this position may be considered in lieu of stated education and experience requirements.
